What You'll Do As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include: Providing personal care with dignity, comfort, and respect at the center of everything Attending to patient needs and requests promptly and compassionately Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed Observing patients and reporting changes to the care team accurately and in a timely manner Communicating with patients and families in a positive, ethical, and confidential manner Maintaining thorough documentation in line with organizational standards Upholding the hospice philosophy with professionalism and genuine compassion Benefits We Offer Competitive pay Company provided equipment Why Choose Transitions Hospice? Requirements What You Bring Active CNA License or Certification in the state of employment Possess and maintains current CPR certification One (1) + year Hospice, home health or related organization experience preferred Comfort and sensitivity working with terminally ill patients and their families A calm, dependable presence under emotionally demanding circumstances Strong attention to detail in observation and documentation A valid driver's license and willingness to travel locally for patient visits
